  2. List 3 words or adjectives you would use to describe yourself.
     

  3. What do you consider your strengths and weaknesses in school and extracurricular performance?
     

  4. Is your highschool academic record (transcript) an accurate measure of your ability and potential? If not, what do you consider the best measure of your potential for success in college work?
     

  5. Please describe the single academic accomplishment (major paper, science experience, math project, artistic project, and/or writing) of which you are most proud. Why do you take special price in this accomplishment? Details are helpful.
     

  6. What special talents do you possess? What skills have you learned or improved upon in activities outside of school? (music, sports, theatrical, etc.) Describe in detail your participation and personal rewards involved. 
     

  7. Which fields/majors might you like to study in college? What are your tentative plans for the future?
     

  8. What are your goals? What do you hope to accomplish in college and for the future? Consider careers and broader life goals — again sharing more thoughts here is better.
     

  9. What would you want to be sure that a college admissions officer knows about you, and what would you like your counselor to highlight?
     

  10. Comment on the influences in your life which have been important to you. (family, friends, sports, teachers, etc.)
     

  11. Ask three teachers who know you well to describe you in three words or adjectives. NOTE: Please include on teacher's name and three adjectives in each box.
     

  12. How has a teacher changed you? Think of a teacher who had an impact on your thinking or behavior because of the classroom experience you had with him/her/them. Describe the experience.
     

  13. Are there any unusual circumstances or problems in your life, in or out of school, that may have emerged, which would help us write about you? How do you want colleges to understand any or all of this?
     

  14. Please note here any additional information that may be helpful in writing your Letter of Recommendation.
